What forms do I file?

You are required to file forms for all lines of insurance, except:

  • Foreign trade insurance
  • Ocean marine
  • Surety insurance
  • Surplus line coverages
Guidance on property and casualty filings
Filings must include:Filings should not include:
  • Policy jackets
  • Declarations pages
  • Renewal certificates
  • Any other form made a part of the policy
  • Only applications made part of the policy
  • Brochures
  • Policyholder notices - unless the notice amends or modifies policy provisions
  • Manuscript forms used on a one-time basis
  • Billing notices, binders, etc
  • Applications, unless made part of the policy
