Follow the rulemaking activity for fire loss reporting (R 2025-03).
Substitute Senate Bill 5419 requires authorized insurance companies to send fire loss reports to the Office of the Insurance Commissioner (OIC). Beginning with claims that close or are subsequently investigated or adjusted on or after January 1, 2026, fire losses must be reported within 90 days after claim closure. Each authorized insurer is also required to complete a fire loss reporting attestation form and submit it to our office by December 31 each year. The first fire loss attestation form is due by December 31, 2026.
This reporting requirement applies to all authorized insurers, as defined in RCW 48.01.050, that write commercial and personal line policies that include coverage for fire loss or damage in Washington state.
Insurers who already report fire loss information through ISO ClaimSearch® do NOT need to send separate fire loss reports or attestation forms to the OIC.

When you fill out the template:
- Don't use commas in any data cells.
- Don't use dollar signs or commas in loss amount cells.
- If applicable, the National Association of Insurance Commissioners (NAIC) number should include leading zeros.

Submitting your fire loss report file
Before submitting your fire loss report file, please rename your file using this format:
fireloss-naicnumber-yyyymm.csv
Replace "naicnumber" with your NAIC number and "yyyymm" with the year and month of your submission. For example, if your company's NAIC number is 12345 and the submission period is September 2026, your file should be renamed "fireloss-12345-202609.csv". The file will fail to process if it's not named correctly.

Fire losses must be filed within 90 days after closing a claim related to fire loss or damage, or within 90 days of any subsequent investigation or adjustment.
