Our office has developed new contract termination notice templates that health care providers and health insurance companies can send to consumers. SSB 5579 required our agency to develop these templates and provide an opportunity for consumers, health care providers, and insurers to submit comments on the proposed templates.
The final templates will be available by December 1, 2025.
How to review and comment
You can download the linked templates and make your suggestions directly in the documents. To submit your comments, email them to OICNetworkaccess@oic.wa.gov with the subject line “Feedback – Health carrier notice” or “Feedback – Provider notice”. Please submit separate comments for each type of notice and combine all feedback from your organization into one email.
Please provide your feedback by the close of business on Monday, November 17, 2025.
