File a complaint or check your complaint status

IMPORTANT:  We do not have authority over insurance policies issued outside of Washington state. Please contact that state's insurance department (www.naic.org) or call us at 800-562-6900.

Complaints against an insurance company

If you have a problem with your insurance company, the OIC can require it to explain its actions and make sure it followed Washington state laws and your policy. Learn more about the complaints process.

Complaints against a provider regarding surprise or balance billing

If you believe you have received a surprise or balance bill from a medical provider or facility, we can request the provider or facility explain its actions and ask them to correct the bill. Learn more about the complaint process.

IMPORTANT: You are protected from surprise billing ONLY if your complaint involves emergency services or a scheduled procedure (PDF, 144KB) at an in-network hospital or in-network surgical facility

You will need to upload your Explanation of Benefits, a signed medical release (PDF, 78KB), and a copy of your medical bill.

Complaints against an agent, agency or other insurance licensee

The OIC investigates complaints against agents, agencies and other insurance licensees. When appropriate we take action, such as imposing fines and suspending or revoking a license. We have no authority to order reimbursement payments, except under very limited circumstances.
