Requirements for certification and notices
There are no specific templates required for long-term care partnership programs, as long as the certification form and notices include all the required elements listed in 48.83 RCW (leg.wa.gov).
Insurers can use templates from the Office of the Insurance Commissioner (OIC) that have all the required elements. Before submitting, download each document and update the information about the insurer, policy, and policyholder.
