Under RCW 48.43.0161, we require certain carriers to submit the annual Prior Authorization Data Report. The data received is used in our annual Prior Authorization Legislative Report.
Who submits prior authorization data
RCW 48.43.0161 Section 1 requires each carrier that has written at least 1% of the total accident and health insurance premiums in Washington to submit a data report. We determine the required companies using premium information available in the NAIC Supplemental Health Care Exhibit.
The following carriers meet the 1% threshold for reporting year 2026 (for 2025 data):
- Aetna Life Insurance Company
- Cigna Health & Life Insurance Company
- Community Health Plan of WA
- Coordinated Care Corp.
- Kaiser Fdn. Health Plan of the Northwest
- Kaiser Fdn. Health Plan of WA Options
- Kaiser Fdn. Health Plan of WA
- LifeWise Health Plan of WA
- Molina HealthCare of WA
- Premera Blue Cross
- Regence BCBS of Oregon
- Regence BlueShield
- UnitedHealthCare Insurance Co.
- UnitedHealthCare of WA Inc.
Note: The list of carriers who will be required to report will be updated annually.
Due dates for your prior authorization data
Each of the carriers listed above must submit a data report to us by September 15, 2026. We will update the template and instructions on this webpage and send a notice to companies in July of each year.
What gets submitted
Each carrier must submit a completed Excel file with each tab filled out by the due date. Each carrier must submit separate files. Submissions representing multiple carriers in a single file are not permitted.
How to submit your prior authorization data
The completed Excel files should be uploaded to Box by the submission deadline.
