A letter of certification is an official document generated by our office confirming the status and history of an insurance license. We generate letters of certification only for our resident licensees.
Request a letter of certification
Send us a request that includes:
- Licensee name.
- WAOIC number.
- How you would like your letter delivered (via postal mail, fax or email).
- Valid postal address, email address or fax number.
- Dated signature (email signature block is acceptable).
- $5 fee (per letter).
Pay by check
Make checks payable to Office of Insurance Commissioner and submit via postal mail.
