Demand For Hearing Instructions
When you receive an Order from our office you have the right to ask for a hearing by submitting a Demand for Hearing. The Demand for Hearing:
- Must be in writing.
- Briefly specify the reason you think the Order should be changed.
- Submitted within 90 days from the date on the order or
- You can waive your rights by letting the 90 days pass.
- Mail your Demand for Hearing to:
- Deputy Commissioner (who signed the order)

- When we receive your request for hearing, we will contact you to:
- Schedule a teleconference to discuss the hearing and the procedures.
- If we recieve your hearing request before the revocation date in the Order, then
- The revocation will be postponed until after the hearing.
