Demand For Hearing Instructions

When you receive an Order from our office you have the right to ask for a hearing by submitting a Demand for Hearing. The Demand for Hearing:

• Must be in writing.
• Briefly specify the reason you think the Order should be changed.
• Submitted within 90 days from the date on the order or
• You can waive your rights by letting the 90 days pass.
• Mail your Demand for Hearing to:
• Deputy Commissioner (who signed the order)
  Office of the Insurance Commissioner
  PO Box 40257
  Olympia, WA 98504-0257
• When we receive your request for hearing, we will contact you to:
• Schedule a teleconference to discuss the hearing and the procedures.
• If we recieve your hearing request before the revocation date in the Order, then
  • The revocation will be postponed until after the hearing.
