The following guidelines will assist you in properly submitting your SERFF filings
in Washington state.
Note: We are now accepting all NAIC product coding matrix types through SERFF.
For life, annuity, credit, long-term care, & medicare supplements
Product coding matrix (145 KB)- Filing Instructions (30 KB)
What's not accepted? Life illustrations, refund calculations, multiple policy reports and grievance reports are not accepted.
For health and disability
- Product coding matrix (145 KB)
- Filing instructions (59 KB)
What's not accepted? Annual Loss Ratio reports and Provider Network Form A reports are not accepted.
For property and casualty
- Product coding matrix (85 KB)
- Form instructions (108 KB)
- Rates and rules instructions (33 KB)
What's not accepted? Motor vehicle service contract filings and credit scoring models are not accepted.
