What forms do I file?
You are required to file forms for all lines of insurance, except:
- foreign trade insurance,
- ocean marine,
- surety insurance,
- surplus lines coverages.
Filings must include:
- policy jackets,
- declarations pages,
- renewal certificates,
- any other form made a part of the policy, and
- only applications made part of the policy.

Filings should not include:
- brochures,
- policyholder notices - unless the notice amends or modifies policy provisions,
- manuscript forms used on a one-time basis,
- billing notices, binders, etc.
- applications, unless made part of the policy.
