What forms do I file?

You are required to file forms for all lines of insurance, except:

  • foreign trade insurance,
  • ocean marine,
  • surety insurance,
  • surplus lines coverages.
Filings must include:

  • policy jackets,
  • declarations pages,
  • renewal certificates,
  • any other form made a part of the policy, and
  • only applications made part of the policy.
Filings should not include:

  • brochures,
  • policyholder notices - unless the notice amends or modifies policy provisions,
  • manuscript forms used on a one-time basis,
  • billing notices, binders, etc.
  • applications, unless made part of the policy.
