Surplus Lines Broker
RESIDENT/NON-RESIDENT BUSINESS ENTITY OR INDIVIDUAL SURPLUS LINE BROKER’S LICENSE
To obtain a business entity or individual surplus line broker’s license, please submit the following:
- Application INS-14 (PDF 101Kb) for Business Entity and Application INS-14A (PDF 101Kb) for the Individual
- License fee in the amount of $200 for the Business Entity, and $200 for the Individual.
- Copy of approved Articles of Incorporation or formation. If a firm, must be registered with the Washington Department of Licensing, (360) 664-1400.
- $20,000 surplus lines broker’s bond in favor of the State of Washington. INS-26 (PDF 18Kb)
- $100,000 surplus lines broker’s bond in favor of the People of the State of Washington INS-64 (PDF 18Kb) OR If already licensed as a broker increase the broker bond INS-17 (PDF 18Kb) to a minimum of $100,000.
- Filing fee in the amount of $5 for the Business Entity, and $5 for the Individual.
- Individual Score Report for Washington Surplus Line Broker exam. Contact Promissor (1-800-274-8949) to schedule exam. Presently the exam is administered once monthly at the Tukwila, Vancouver, Spokane and Lacey, Washington test sites. Arrangements can be made with Promissor to administer the exam at other locations for an additional fee.
The minimum requirements to be met by an applicant before he or she will be permitted to take the surplus line broker examination are:
An applicant must have been licensed as a casualty-property broker for not less than five years preceding the date of the application, or have received the chartered property casualty underwriter (CPCU) designation with not less than five years' experience in the insurance industry preceding the date of the application, or have not less than ten years' experience as an insurance company employee, or an employee of an insurance broker's office or other related insurance industry experience preceding the date of the application, or have other equivalent experience acceptable to the insurance commissioner
- Completed affiliation notice INS-18 (PDF 13Kb).
- Affiliation fee in the amount of $20 (unless already affiliated as an agent and/or a broker).
- If non-resident, Letters of Certification for Surplus Line Broker license from the resident state for the business entity and the individual(s).
- One fingerprint card, Form FD-258, for each individual currently not licensed in Washington and a check in the amount of $54 per individual, payable to the Washington State Patrol - ID Section.
** There will be two separate Surplus Line Broker licenses issued; one for the Individual and one for the Business Entity. **
