You can apply for a license using OIC online services or submit a paper application by following the instructions for printable forms below.

Apply using printable forms - application requirements
- Your examination score report(s) from PearsonVue.
- You may go to the PearsonVue website or call them: 1-800-274-8949.
- Your fingerprints. See fingerprinting requirements.

What if I represent a business entity?
Is the business entity already licensed as a surplus lines broker?
- Yes. Submit the Affiliation notice form (59 KB)
- No. The business entity must obtain a surplus lines broker license. See instructions for business entity licensing.
Please note:
You'll need to have your surplus lines broker bonds in place prior to placing business.
Paying your surplus lines taxes
Remember to pay your surplus lines taxes by March 1st.
Fee Schedule
Please make checks payable to: Washington Insurance Commissioner. You may combine all fees in one check.
- License fee: $200.
- Filing fee: $5.*
- Fingerprint processing fee: $45.25*
- Affiliation fee: $20*
*Do not pay if you're already licensed or affiliated as a producer.
