Variable life & annuity - applying for a license
For individual producers who live in Washington

You can apply for a license using the OIC online licensing services or submit a paper application by following the instructions below.

(Please note, prior registration is necessary for utilizing the online option.)
Apply using printable forms
Please submit the following information via fax or email.
- Request to add variable LOA to your license
- Your WAOIC number
- Your current securities license number (the CRD number). This is obtained through the Financial Industry Regulatory Authority (FINRA) or the WA Department of Financial Institutions (DFI).
