Request to add variable LOA to your license
Affiliation notice form (59 KB)
- You must affiliate at least one individual who is licensed in WA for insurance and securities
Your current securities license number (the CRD number)
- Contact FINRA at http://www.finra.org/index.html OR DFI at http://dfi.wa.gov/sd/brokerdealer.html for securities registration information.
