Premium Finance Company

To obtain a resident or non-resident Premium Finance Company License, please submit the following:

  1. Firm or Corporation Insurance License Application - (INS-14 APF) (PDF 62Kb), completed on behalf of the company.
  2. Firm or Corporation Insurance License Application - (INS-14 APF) (PDF 62Kb), completed by every producer that is financing premiums or signing contracts/agreements.
  3. License fee of $100.00.
  4. Filing fee of $5.00.
  5. A Fingerprint Card (FD-258) including $49.25 payable to the Washington State Patrol Identification Section. You may use a cashier’s check, company check or money order.
  6. Copy of Articles of Incorporation.
