Public adjusters applying for a license
For business entities not located in Washington
You can apply for a license using the OIC online licensing services or submit a paper application by following the instructions for printable forms below.

IMPORTANT: There are additional instructions for completing the uniform application in Washington state:
At the top of the, "Jurisdiction and Type of License/Registration Requested" page there is key for abbreviations in the grid.
Just below these abbreviations, there are categories for Jurisdiction, Legal Business Type, License/Registration Type and Lines of Authority (Major Lines or Limited Lines).
On the, "Limited Lines of Authority" page:
- Find the WA grid line in the, "Jurisdiction" column.
- In the, "Limited Lines of Authority" section, on the WA grid line, please write, "Public Adjuster" in the, "Other" column.
Public adjuster's bond - INS-21 (18 KB). In the amount of $5,000.- Affiliation notice form (59 KB).
- At least one individual adjuster must be affiliated.
- For corporations or LLCs, send a copy of your Articles of Incorporation or formation.
- For firms, file your trade name with the Department of Licensing, 360-664-1400. A firm includes a partnership or sole proprietorship.
Fee Schedule
Please make checks payable to: Washington Insurance Commissioner. You may combine all fees on one check.
- License fee: $50 for each location
- Filing fee: $5
- Affiliation fee: $20
