IMPORTANT: There are additional instructions for completing the uniform application in Washington state. Please be sure to indicate what capacity you will be operating under.

- Agency must affiliate at least one individual who is licensed in WA for both insurance and securities.
Uniform appointment form (42 KB)
- You may submit the appointment with your application packet or
- your carrier may submit electronically within 15 days from your first piece of business.
- For firms, file your trade name with the Department of Licensing at 360-664-1400. A firm includes a partnership or sole proprietorship.
Fee Schedule
Please make checks payable to: Washington Insurance Commissioner. You may combine all fees in one check.
- License fee: $55 for each location.
- Filing fee: $5.
- Affiliation fee: $20*
- Appointment fee: $20*
*Do not pay this fee to us if you’re not including the form(s) with your other application materials or if it will be submitted electronically.

Uniform application for business entity insurance license
- At the top of the, “Jurisdiction and Type of License Requested” page there is a key for abbreviations in the grid.
- Just below these abbreviations, there are categories for Jurisdiction, Legal Business Type, License/Registration Type and Lines of Authority.
For Agent For Broker
- Find the WA grid line in the Jurisdiction column.
- In the, “Legal Business Type” column, on the WA grid line, place an X in the column that identifies your business’s articles of incorporation.
- In the, “License/Registration Type” column, on the WA grid line, place an X in the P (for producer) column. You will also need to place an X in the A (for agent) column.
- In the, “Lines of Authority” column, on the WA grid line, select the lines of insurance your business is planning on selling. Place an X in each line you are requesting on your license.
- The grid on the, “Jurisdiction and Type of License/Registration – Limited Lines of Authority” page need only be completed if your business is planning on selling Limited Lines, following the instructions above.
- Find the WA grid line in the Jurisdiction column.
- In the, “Legal Business Type” column, on the WA grid line, place an X in the column that identifies your business’s articles of incorporation.
- In the, “License/Registration” type column, on the WA grid line, place an X in the P (for producer) column. You will also need to place an X in the B (for broker) column.
- In the, “Lines of Authority” column, on the WA grid line, select the lines of insurance your business is planning on selling. Place an X in each column for each line you are requesting on your license.
