You can apply for a license using the OIC online licensing services or submit a paper application by following the instructions for printable forms below.
IMPORTANT: There are additional instructions for completing the uniform application in WA. Please be sure to indicate what capacity you will be operating under.
If you will be operating as... an Agent a Broker What does this mean? What does this mean? You will need to indicate this on the, “Jurisdiction and Type of License Requested” page of the uniform application.

Affiliation notice form (59 KB)
- You must affiliate at least one licensed individual.
REMEMBER: An individual producer cannot transact business for you until the affiliation has been received and confirmed by our office.
Uniform appointment form (42 KB)
- You may submit the appointment with your application packet or
- your carrier may submit electronically within 15 days from your first piece of business.
For firms, file your trade name with the Department of Licensing at 360-664-1400. A firm includes a partnership or sole proprietorship.
Please make checks payable to: Washington Insurance Commissioner. You may combine all fees on one check.
*Do not pay this fee to us if you’re not including the form with your other application materials or if it will be submitted electronically.
