Licensing Renewal Procedures

Initial Licensing

If you receive a passing score on your insurance exam, forward the following materials to the Office of Insurance Commissioner:

  • An Individual Insurance License Application - (INS-14) (PDF 102Kb)
  • An Appointment/Affiliation Notice- (INS-18) (PDF 13Kb) completed by each insurer represented or
    • If applicable, an Appointment/Affiliation Request For - (INS-18) (PDF 13Kb). Use if you are:
      • Representing a business entity - An affiliation with the business entity is required.
      • Directly representing an insurer - The appointment is due at time of application unless it's submitted electronically through the National Insurance Producer Registry (NIPR) within 30 days of the acceptance of the first insurance application executed by the agent with the insurer
  • A Fingerprint Card (FD-258) including $49.25 payable to the Washington State Patrol Identification Section. You may use a cashier’s check, company check or money order.
  • Score Reports
  • License Fees payable to the Washington Insurance Commissioner
    • If the appointment is being done electronically through NIPR, fees are to be submitted to them. Do not send $20.00 to our office for the appointment if it is being processed through NIPR.
  • If for a broker license:

Applicants for a resident license as a life, disability, property or casualty insurance agent or solicitor must also include validated Certificates of Completion of the approved pre-license education or a pre-license education waiver, if applicable.

The upper portion of qualification license will contain the licensee’s unique Identification Code (PIC). The license itself has your WAOIC#. It is very important that you use one of these identifying numbers in all future correspondence.

Applicants who are not licensed within two years after passing an examination must be reexamined.

Agents and brokers holding valid insurance licenses, who have passed an examination qualifying them to market an additional insurance line(s) must submit the score report and pre-licensing Certificates of Completion of the approved pre-license education or a pre-license education waiver, if applicable. The licensee will be furnished an updated license and no fee is required.

License Renewal

All individual licenses, regardless of the issue date, will expire on the individual’s next birth date, plus 1 year, and every 2 years thereafter.

Firm and corporate licenses expire two years from the date issued and every two years hereafter. The business entity must also renew its affiliations at time of renewal.

A renewal notice will be sent to the licensee’s last address of record approximately 45 days prior to the license expiration date. Invalid addresses are the number one reason why agents do not receive their renewal form. If you have not received your renewal 30 days prior to the expiration date of your license, please call the Licensing & Education Section, 360-725-7144 and request a duplicate. A duplicate will be mailed to you and late fees will apply. It is the licensee’s obligation to renew prior to the expiration date and the renewal form is required when renewing the license. If a license is not renewed, appointments and affiliations will be terminated. The Office of Insurance Commissioner notifies insurers and firms/corporations of the termination.
