Application Procedures
The following is a list of all types of insurance licenses issued by the State of Washington and the forms and fees that must be submitted to apply for each type of license.
RESIDENT—INDIVIDUAL
Agent - Life, Disability, Property or Casualty
Forms:

- Appointment and/or Affiliation Request Form INS-18 (PDF 13Kb)
(Applicant must be affiliated to a licensed agency if he/she represents and/or shares commissions with the agency or must be appointed if he/she directly represents an insurer.)- If you are representing a business entity, an affiliation with the business entity is required.
- If you are directly representing an insurer, the appointment is due at time of application unless the appointment is going to be submitted electronically through the National Insurance Producer Registry (NIPR) gateway within thirty (30) days of the acceptance of the first insurance application executed by the agent with the insurer.
- Pre-License Education Certificates of Completion
- Examination Score Report(s)

- License Fee $50
- Filing Fee $5
- Appointment/Affiliation Fee $20 each

All fees may be combined into one check payable to the Washington Insurance Commissioner.
Agent - Surety, Vehicle

- If you are representing a business entity, an affiliation with the business entity is required.

with the business entity whose bond is sufficient to include applicant ($20,000 covers corporation or firm and one broker, $5,000 additional for each broker) )
(Applicant must be affiliated to a licensed agency if he/she represents and/or shares commissions with the agency)

RESIDENT—BUSINESS ENTITY (FIRM, CORPORATION, LLC)
Additional Branch location fee within Washington is the same as the license(s) fee for the Main location
Agent
